To evaluate the safety and efficacy of an abbreviated course of antibiotic therapy in postpartum endomyometritis, 109 patients with endomyometritis were randomized to three study groups. All were treated with clindamycin and tobramycin until afebrility and clinical signs of disease were absent. Patients in group I received antibiotics for greater than or equal to 24 hours, group II received therapy for greater than or equal to 48 hours, and group III received antibiotic therapy for greater than or equal to 48 hours that preceded a 7-day course of oral Augmentin. Six patients with pseudomembranous colitis unrelated to lincomycin or clindamycin treatment were investigated for signs of localised or diffuse cytomegalovirus (CMV) infection. Testing for CMV included rectal mucosa biopsies, culture of blood leucocytes with human embryo diploid fibroblasts in continuous layer and titration of complement-deviating anti-CMV antibodies. Cytomegalic cells with nuclear inclusion bodies were found in the rectal mucosa of 5 patients, 4 of whom also had foci of CMV-infected cells in leucocyte-fibroblast cultures, indicative of viraemia. The fifth patient was not tested for viraemia but developed very high anti-CMV antibodies titers at a later stage. These results show that pseudomembranous colitis that are not due to antibiotics are frequently associated with localised or diffuse CMV infection. Viral invasion of the colon might be encouraged by a state of immunodeficiency.

Objective. A combination of intravenous clindamycin and oral tetracycline has been used for many years as a treatment for active rheumatoid arthritis (RA), despite the absence of good evidence for its efficacy. A single-blind pilot study of this therapy suggested that a double-blind placebo-controlled trial was warranted. Methods. Patients with active RA were randomised in a 2 : 1 ratio to receive active treatment or placebo for 25 weeks. The active treatment consisted of intravenous clindamycin in a reducing regime, and oral tetracycline twice daily three times a week. 50 patients were to be recruited. The primary outcome measure was the proportion of patients achieving an ACR20 response. Results. An interim statistical analysis was performed after 20 patients had completed the study. Two patients in the active group achieved an ACR20 response, with none in the placebo group (NS). There was a better ESR20 response in the placebo group (P = .02). There were no other significant differences between the groups. The results indicated that it was unlikely that a significant difference in ACR20 response would emerge if the remaining 30 patients were recruited. The trial was therefore halted. Conclusion. This antibiotic regime is unlikely to be a valuable therapy for active rheumatoid arthritis.

Bacterial vaginosis, the most common infectious cause of vaginitis, is characterized by a shift in normal vaginal flora from predominantly aerobic to mainly anaerobic flora. Incidence rates in various studies have ranged from 1-0-45%. Unclear are both the pathophysiology of the observed reduction in lactobacilli and whether bacterial vaginosis is sexually transmitted. Evidence for sexual transmission is provided by a significantly greater incidence of bacterial vaginosis in women with more than 5 sexual partners compared to those in a lifelong monogamous relationship, while bacterial detection in virgins and the failure to demonstrate benefits of partner treatment argue against sexual transmission. Use of an IUD does appear to be a risk factor. The only symptom consistently reported by women with bacterial vaginosis is a malodorous vaginal discharge; however, over half of women with this condition are asymptomatic. The standard criterion for diagnosis includes the presence of 3 of the following signs: the clue cell, homogeneous discharge that adheres to but is easily wiped off the vaginal wall, an elevated vaginal pH, and the potassium hydroxide test for volatile amines. Of concern is the potential for serious sequelae. The abnormal bacterial flora associated with vaginosis significantly impede the white blood cell response to infection. Gynecologic-obstetric sequelae include pelvic inflammatory disease, endometritis, postoperative vaginal cuff infections, preterm labor, premature rupture of membranes, and chorioamnionitis. Metronidazole and clindamycin are the most effective treatment agents. Given the long-term risks associated with bacterial vaginosis, a full course of antibiotics should be considered in both obstetric patients with this condition and women undergoing outpatient ambulatory invasive surgical procedures.
Although the exact role of quorum sensing (QS) in various stages of biofilm formation, maturation, and dispersal and in biofilm resistance is not entirely clear, the use of QS inhibitors (QSI) has been proposed as a potential antibiofilm strategy. We have investigated whether QSI enhance the susceptibility of bacterial biofilms to treatment with conventional antimicrobial agents. The QSI used in our study target the acyl-homoserine lactone-based QS system present in Pseudomonas aeruginosa and Burkholderia cepacia complex organisms (baicalin hydrate, cinnamaldehyde) or the peptide-based system present in Staphylococcus aureus (hamamelitannin). The effect of tobramycin (P. aeruginosa, B. cepacia complex) and clindamycin or vancomycin (S. aureus), alone or in combination with QSI, was evaluated in various in vitro and in vivo biofilm model systems, including two invertebrate models and one mouse pulmonary infection model. In vitro the combined use of an antibiotic and a QSI generally resulted in increased killing compared to killing by an antibiotic alone, although reductions were strain and model dependent. A significantly higher fraction of infected Galleria mellonella larvae and Caenorhabditis elegans survived infection following combined treatment, compared to treatment with an antibiotic alone. Finally, the combined use of tobramycin and baicalin hydrate reduced the microbial load in the lungs of BALB/c mice infected with Burkholderia cenocepacia more than tobramycin treatment alone. Our data suggest that QSI may increase the success of antibiotic treatment by increasing the susceptibility of bacterial biofilms and/or by increasing host survival following infection.
