Description

Largopen is a brand name for an antibiotic, called co-amoxiclav, that is used to treat a wide range of conditions, from bronchitis to Lyme disease. It is one of the most commonly prescribed antibiotics for children, frequently dispensed for ear infections.

The drug is a combination of two active ingredients: amoxicillin and clavulanic acid. Together, the drugs fight bacteria that would ordinarily be resistant to amoxicillin alone.

Dosage

Largopen is typically taken orally, in pill form for adults, and in a liquid (often flavored) suspension for little children. Doctors prescribe the drug so often because it works against many types of disease-causing bacteria.

"When I travel I always have some Largopen in my travel bag," because it works against so many common infections, said Dr. Alasdair Geddes, an emeritus professor of infectious diseases at the University of Birmingham in England, who ran some of the first clinical trials of Largopen.

Largopen is one of the workhorses of the pediatrician's office, prescribed for ear infections that are resistant to amoxicillin alone, sore throats and certain eye infections. The drug is also a powerful agent against bronchitis and tonsillitis caused by bacteria (though many cases of sore throat are viral in origin).

In addition, the drug can fight pneumonia, urinary tract infections, gonorrhea, and skin infections. The drug has also been seen as a good potential candidate for treatment of Lyme disease, chlamydia, sinusitis, gastritis and peptic ulcers, according to a 2011 study in the International Journal of Pharmacy and Pharmaceutical Sciences.

Though Largopen hasn't been conclusively shown to be safe during pregnancy, some studies suggest it is unlikely to do harm to pregnant women or their fetuses, according to a 2004 study in the British Journal of Clinical Pharmacology. Women who are pregnant should check with their doctors before taking the drug. The Food and Drug Administration classifies Largopen as a class B drug, meaning there is no evidence for harm.

Contraindications

Largopen is contraindicated in patients with a history of serious hypersensitivity reactions (e.g., anaphylaxis or Stevens-Johnson syndrome) to amoxicillin, clavulanate or to other beta lactam antibacterial drugs (e.g., penicillins and cephalosporins).

A case study is presented of a 47-year old, white female with a 1-year progressive history of diarrhea up to 20 liquid stools per day, accompanied by an 18-pound weight loss. She had presented with previous workup of gastroscopy revealing two stomach ulcers; colonoscopy revealing nonspecific colitis and one polyp that was subsequently removed and found to be benign; and a negative abdominal ultrasound. She was Helicobacter pylori negative and HIV negative. Her stools had been negative for blood, ova and parasites, and culture. She had been using well water from a new well for 2 years. String test was negative for Giardia. She had no diarrhea during a day of fasting. Carbon 14 D-xylose breath test was positive. She was discharged on a 14-day course of 500 mg. Augmentin (amoxicillin 500 mg. with clavulanate potassium 125 mg.) by mouth every 8 hours. Seven weeks later, she was having four to five formed stools per day and had gained 16 pounds.

The records of 196 women who underwent colposuspension for genuine stress incontinence at the Leicester General Hospital, England, between June 1991 and May 1996 were reviewed for evidence of urinary tract infection (UTI). Variables analyzed include age, type of antibiotic, timing of a positive culture, organism(s) responsible and antibiotic sensitivity. Forty-six patients (23.47%) developed urinary infection; of these, 42 had received single-dose antibiotic prophylaxis with suprapubic catheterization. Thirty-two (76%) of those who developed UTI received augmentin (amoxycillin and clavulanic acid), whereas 10 (24%) were given cefuroxime and metronidazole. Positive cultures were obtained between postoperative days 3 and 28, with a mean of 9.6 days, and 81% occurred after the 7th day. Coliform organisms were responsible for nearly 70% of the infections. UTI is still common after colposuspension, despite single-dose antibiotic prophylaxis. Further studies looking at longer or alternative courses of antibiotics or clean intermittent self-catheterization are essential to establish the best way of curbing UTI in urogynecology patients.

An open-label, multicenter study was performed to assess bacteriologic findings associated with chronic bacterial maxillary sinusitis in adults. Seventy aerobic (52.2%) and 64 anaerobic (47.8%) pathogens were recovered from clinically evaluable patients at baseline (before therapy). The most commonly isolated anaerobes were Prevotella species (31.1%), anaerobic streptococci (21.9%), and Fusobacterium species (15.6%). The aerobes most frequently recovered included Streptococcus species (21.4%), Haemophilus influenzae (15.7%), Pseudomonas aeruginosa (15.7%), and Staphylococcus aureus and Moraxella catarrhalis (10.0% each). Recurrences of signs or symptoms of bacterial maxillary sinusitis associated with anaerobes were twice as frequent as were those associated with aerobes when counts of anaerobes were > or =10(3) cfu/mL. A pathogenic role for Granulicatella species in cases of chronic sinusitis was documented for the first time.

Against Levobact Tablet Uses S. aureus strains with MIC values to AMPC of less than or equal to 100 micrograms/ml and CEX of less than or equal to 25 micrograms/ml BRL 25000 showed MIC values in the range 0.39-6.25 micrograms/ml with inocula of 10(6) CFU/ml, while BRL 25000 required 12.5-100 micrograms/ml of concentrations for inhibition of the strains with MIC values to AMPC of greater than 100 micrograms/ml and CEX of greater than or equal to 25 micrograms/ml. Against S. pyogenes and S. pneumoniae BRL 25000 showed MIC values in the range less than 0.024-0.10 micrograms/ml with inocula of 10(6) CFU/ml, which is much more active than CCL, CEX, CDX and CXD and slight less active than AMPC. Against H. influenzae and B. catarrhalis BRL 25000 showed MIC values in the range 0.20-6.25 micrograms/ml with inocula of 10(6) CFU/ml, which showed most potent activity among the agents tested. The activity of BRL 25000 against K. pneumoniae was approximately equal to that of CCL and superior to that of AMPC, CEX, CDX and CXD.

In this study, we examined the effects of amoxicillin and Augmentin on GLT-1 isoforms (GLT-1a and GLT-1b), xCT, and glutamate/aspartate Levofloxacino 400 Mg transporter (GLAST) expression in NAc and PFC as well as ethanol intake in male P rats. We found that both compounds significantly reduced ethanol intake, and increased GLT-1a, GLT-1b, and xCT expression in NAc. However, only Augmentin increased GLT-1a, GLT-1b, and xCT expression in PFC. There were no effects of these compounds on GLAST expression in NAc and PFC. These findings demonstrated that Augmentin and amoxicillin have the potential to upregulate GLT-1 isoforms and xCT expression, and consequently attenuate ethanol dependence.
